We have provided you with instructions below to assist you with the online registration process:

Upon clicking your clinician’s button above, you will be taken to their individual web page. You will then need to click on the “Register as a new patient/client” button to create a new client account.

Please fill out your personal information in this section. If you are not using insurance, feel free to leave that section blank.

 

You will be required to establish a login name and password. This will allow you to access your clinician’s web page in the future to obtain your billing statements, change personal information, schedule appointments, etc.

Following the completion of your personal information, you will be asked to complete the biographical information section. Please complete this document prior to attending your first appointment.

At the end of your online registration process, you will be given the option to schedule with your clinician. To do so, click the “Set Appointment” button and select an available date/time.

Scheduling Appointments by Phone

If you prefer you may schedule an appointment via phone by calling the office at 630-686-2800. If you do not reach a live person, please leave a voicemail with your name, phone number, and some indication about what days and times you would be available to meet.

If you would like to speak with a potential clinician prior to scheduling an appointment, please email her directly and she will be happy to contact you via phone or email to discuss the possibility of working together to address your concerns.
